Birth Certificates: If you are inquiring about getting a copy of a birth certificate, please visit Florida Department of Health's website for more information. Medical Records: If you would like to request a copy of your medical records, please contact the Health Information Management Department at (850) 278-3020. One of our representatives will be happy to assist you.
